I have just begun adding attachments such as PPT slides to the sermons that I post. Is there any way to add a label to them so that people see something other than just a PDF icon? It would be nice to be able to put some text next to it that says "Sermon Notes" or something like that so people know what the PDF icon is there for.

If you aren't using PDF's, you could just use an image editor to change the icon graphic. We did that as we link to a post version of the text… so I enter it as a URL, and changed the URL icon graphic to a plain page that says "Read" on it. Works pretty well without dealing with the code.

Any other suggestions? It is quite confusing when there are two attachments, e.g., Sermon Text + Small Group Questions. Would love just to have a text link that says "Download Small Group Questions" etc.

Version 0.46 will allow you to show the filename next to the icon. It won't be perfect, but it might help.

There are some fairly major structural changes in 0.46 to support new template features and pretty permalinks, so I'm likely to release a beta first. I'll probably do that within a few days. If there's only minor problems, then the full version will be another week after that. If there are major problems, then of course it will be delayed.
